Enrolment Process
Before enrolling, please refer to our Practice Fees page for a list of our fees and incurring charges.
Enrolling with us is easy; simply follow the instructions below:
By enrolling you confirm that you intend to use Onewa Doctors as your regular and ongoing GP and that you intend to reside permanently in New Zealand for more than 183 days in the next 12 months.
Click on this link for the ONLINE ENROLMENT FORM. This is the recommended, secure and easiest way to enroll. Please note, you will still be required to upload proof of ID and eligibility.

Download, print and complete the manual form as per instructions below:
- Download and fill in the ENROLMENT FORM PACKAGE
- Scan a copy of the following for proof of eligibility:
- Patients born in NZ: Passport or Birth Certificate or Gold card
- Patients born outside of NZ: Patients are subject to the Ministry of Health Eligibility Criteria – Passport and Visa (all visas i.e. Student, Work, Permanent Resident)
- The completed enrolment forms, relevant documents and identification above can either be scanned and emailed to reception@onewadoctors.com or nurse@onewadoctors.com; or dropped off at reception (Please note that we cannot guarantee the safety of your information when submitted via email, thus we recommend dropping your manual forms to Reception)
Failure to submit ALL forms and proof of eligibility may cause a delay in processing your enrolment.
